On your first visit, please arrive 10-15 minutes early to complete the necessary paperwork.

Bring the following to your appointment:

  • Physician prescription, if you have one

  • Insurance card

  • Completed Medical History Form

  • Worker’s compensation paperwork, if applicable

Information can be brought to the clinic at the time of your appointment or emailed or faxed to your preferred clinic.
